Thermometer: does it work?

For fever, digital oral or rectal readings are the most reliable; ear and forehead models are faster but miss more fevers. 'Normal' isn't exactly 37°C.

What it is

Home thermometers come in two broad kinds. Digital contact thermometers (oral, underarm, rectal) read the temperature where the tip sits. Infrared thermometers read heat radiated from the eardrum or forehead: fast and easy on a sleeping child, but more sensitive to technique, sweat, room temperature and distance. Basal thermometers read to two decimal places for cycle tracking.

A fever is generally 38.0°C (100.4°F) or higher. 'Normal' varies: in 35,488 clinic patients, average oral temperature was 36.6°C, and individuals had their own baselines.

How strong is the evidence?

Taking a temperature is simple, but the site matters. In a meta-analysis of 75 studies, mostly in hospital patients, ear, forehead, underarm and oral thermometers often differed from core temperature by more than 0.5°C and caught only about 64% of fevers, though they rarely reported fevers that weren't there. In children, ear readings differed from rectal ones by up to about 1°C either way. Non-contact forehead devices picked up about 81% of fevers in a separate meta-analysis. For babies, a rectal digital reading remains the reference.

What to look for

  • For fever: a digital thermometer for oral, underarm or rectal use; for babies, one with a flexible tip suitable for rectal readings.
  • For speed with children: an ear or forehead model used exactly as the manual says, with a digital thermometer to confirm when it matters.
  • The age range the device is designed for; ear thermometers aren't recommended for babies under about 6 months.
  • A clear display, a fever alert and memory for recent readings.
  • For cycle tracking: a basal thermometer that reads to 0.01°C, or a wearable that tracks overnight skin temperature.
  • Easy cleaning: probe covers or a washable tip. Label a rectal thermometer and never use it by mouth.

What to skip

  • Mercury glass thermometers. They break and spill mercury, and digital ones are just as good.
  • Phone apps that claim to take your temperature without a sensor.
  • App features that promise to predict illness or fertility from a single reading.

How to use it

  • Wait 15–30 minutes after eating, drinking or exercise before an oral reading.
  • Follow the positioning instructions exactly for ear and forehead models, and repeat the reading if it seems off.
  • For basal temperature, measure on waking, before getting up, at the same time each day.

Who should be careful

  • Get medical care right away for a baby under 3 months with a temperature of 38.0°C (100.4°F) or higher.
  • At any age, seek urgent care for a fever with a stiff neck, severe headache, confusion, a rash that doesn't fade when pressed, trouble breathing, or a seizure.
  • See a clinician for a fever that keeps climbing or lasts more than a few days.
  • Symptoms outrank the number: someone who looks very unwell with a 'normal' forehead reading still needs care.
  • Don't rely on temperature-based cycle tracking as contraception without discussing its typical-use failure rate with a clinician.

    In 31 comparisons covering 4,441 children, ear readings averaged 0.29°C below rectal ones, but individual differences ranged from about −0.7 to +1.3°C, too wide for situations that need precise measurement.
