Watch or fitness tracker: does it work?

Wrist devices that log heart rate, steps and sleep. Good for resting heart rate and movement; rough for calories, VO₂ max and sleep stages.

What it is

Wrist trackers and smartwatches use motion sensors and optical heart-rate sensors (light shone into the skin to detect each pulse) to estimate steps, heart rate and sleep, and on some models blood oxygen, skin temperature, fitness (VO₂ max) and heart rhythm. A few features, such as an ECG app or irregular-rhythm notifications, may be cleared by regulators; most of the rest are wellness estimates never reviewed for medical accuracy.

The value is in trends against your own baseline: resting heart rate, daily movement, and when you sleep.

How strong is the evidence?

Moderate evidence

An umbrella review of 39 meta-analyses (about 164,000 people) found activity trackers raised physical activity by roughly 1,800 steps a day, with small effects on weight and fitness. In a study of 419,297 smartwatch users, 34% of those who got an irregular-pulse alert had atrial fibrillation confirmed on a later ECG patch. Accuracy depends on the metric: heart rate is good at rest and worse during movement, no tested device estimated calories within 20%, and VO₂ max estimates can be off by about ±10 mL/kg/min for an individual.

Cuffless readings aren't recommended for diagnosing or managing blood pressure. Use a validated upper-arm cuff.

Tracks your deep and REM sleep

Timing and total sleep are fair; stage breakdowns are inconsistent, and sleep or 'readiness' scores aren't validated clinical measures.

What to look for

  • Independent validation studies for the metrics you care about (search the model name plus 'validation'), not the marketing page.
  • Comfort and battery life you'll live with; gaps break trends.
  • Regulatory clearance for any medical-style feature you'll rely on, such as ECG or irregular-rhythm alerts. Clearance applies feature by feature.
  • Data export or syncing to your phone's health app, so years of history aren't trapped if you switch.
  • Any subscription cost, and a privacy policy that lets you delete your data and limits sharing with advertisers.
  • If exercise heart rate matters to you (intervals, zones), plan to pair it with a chest strap.

What to skip

  • Calorie-burn numbers as a basis for how much to eat.
  • 'Stress', 'body battery' and readiness scores as verdicts on your health; they're proprietary blends.
  • Cuffless blood pressure features.
  • Paying extra for sensors whose readings you won't act on.

How to use it

  • Learn your baseline over two to three weeks before reading anything into a number.
  • Review weekly averages, not daily scores.
  • Wear it the same way (same wrist, same snug fit) so trends stay comparable.

Who should be careful

  • An irregular-rhythm alert is a reason to book an ECG. A watch can't detect a heart attack: call emergency services for chest pain, fainting, severe breathlessness or signs of stroke, whatever the watch says.
  • Blood-oxygen readings from light sensors can overestimate oxygen on darker skin, and wrist SpO₂ is a wellness feature. Symptoms outrank the number.
  • See a clinician if your resting heart rate is consistently above 100, or below 50 with dizziness, fainting or unusual fatigue.
  • If tracking makes you anxious or you find yourself chasing sleep scores, hide the scores or take a break.
  • Keep the skin under the band clean and dry, and loosen it if it irritates.

    Pooling 39 systematic reviews covering 163,992 people, activity trackers increased physical activity by about 1,800 steps a day and reduced body weight by about 1 kg; effects on blood pressure, cholesterol and HbA1c were small and often not significant.

    Of 419,297 smartwatch wearers, 0.52% received an irregular-pulse notification; among those who then wore an ECG patch (on average 13 days later), 34% had atrial fibrillation, and 84% of alerts during patch wear coincided with it. The study was funded by the device maker.

    In 60 volunteers tested against ECG and indirect calorimetry, six of seven wrist devices measured heart rate within 5% during cycling, but none estimated energy expenditure within 20%. Errors were larger during walking and with higher BMI and darker skin.
