Supports & braces: does it work?

Knee and elbow sleeves, ankle and wrist braces, back supports and compression sleeves. Ankle braces have real evidence; most of the rest is for comfort.

What it is

Sleeves (neoprene or knitted) add warmth, compression and a feeling of support. Braces go further: lace-up or strapped ankle braces limit rolling, wrist and shoulder braces restrict movement, and back supports wrap the lower trunk. Compression sleeves for calves and arms apply firm pressure.

Start with what problem you're solving. Preventing a repeat ankle sprain is well supported. Making a sore joint feel better while you train is reasonable. Preventing injury in a healthy knee, elbow or back hasn't been shown.

How strong is the evidence?

Moderate evidence

Randomized trials support ankle braces: in 1,460 high-school basketball players, lace-up braces cut acute ankle injuries by about two-thirds, and reviews find strong evidence that bracing prevents repeat sprains. A Cochrane review found moderate evidence that back supports don't prevent back pain. Across 23 studies, compression garments showed small-to-moderate benefits for recovery, but those studies can't be blinded. Sleeves for healthy knees and elbows are barely studied.

Ankle braces prevent sprains

Holds up

In a trial of high-school basketball players, lace-up braces cut acute ankle injuries by about 68%, whether or not players had sprained before. Injuries that did happen were no less severe.

Back support belts prevent back injury

False

A Cochrane review found moderate evidence they're no better than no intervention for preventing back pain, and a study of nearly 14,000 workers found no fewer injuries.

Knee sleeves prevent knee injuries

Unproven

Warmth and compression can make knees feel better under load, but no good trials show fewer injuries in healthy lifters.

Compression sleeves speed recovery

Mixed

A meta-analysis found small benefits, larger for strength recovery after resistance training. Participants always know they're wearing compression, and effects over weeks of training are unknown.

Copper-infused fabric relieves pain and inflammation

Unproven

There's no good evidence for it. In a randomized trial of copper bracelets for rheumatoid arthritis, they did no better than placebo. Any effect of a copper sleeve comes from the compression.

What to look for

  • A product matched to the problem. Sprain-prone ankles in court or field sports: a lace-up or strapped ankle brace. A knee that aches during squats: a sleeve for comfort. Pain after an injury: get it assessed before you buy anything.
  • Size by measurement. Sleeves and braces are sized by circumference at a set point (around the knee, calf or elbow). Too tight cuts off circulation; too loose does nothing.
  • Thickness for the job. Thick (5–7 mm) neoprene knee sleeves are warmer and stiffer for squatting. Thin knitted sleeves suit running and everyday wear.
  • Ankle braces that fit inside your shoe without bunching. Lace-up braces with straps give the most support.
  • Compression sleeves: correct sizing and a firm, comfortable fit. Pressures beyond that haven't been shown to add benefit.
  • Washable, breathable material. Sleeves you wear often get sweaty; machine-washable ones last.

What to skip

  • Copper, magnetic or 'infrared-infused' fabrics: no good evidence they add anything to plain compression.
  • Back support belts for everyday lifting or all-day wear: they don't prevent back pain.
  • Braces for a healthy joint 'just in case': apart from ankle braces in sprain-prone sports, they haven't been shown to prevent injury.
  • Medical-sounding claims on sports sleeves: they're comfort products, not treatment.

How to use it

  • After an ankle sprain, do balance and strengthening exercises as well as wearing a brace for sport. Bracing alone doesn't restore the ankle.
  • Wear sleeves for heavy sessions rather than all day.
  • Put knee sleeves on before you warm up; they're easier to fit then and add warmth.

Who should be careful

  • Pain that persists, swelling, instability, locking or numbness needs assessment by a clinician or physiotherapist. A sleeve or brace can mask a problem that needs treatment.
  • Sleeves, straps and compression garments that are too tight can cut off circulation. Numbness, tingling, colour change or swelling below the garment means loosen or remove it.
  • If you have peripheral artery disease, severe neuropathy (for example from diabetes), fragile skin or heart failure, ask a clinician before using compression garments.
  • Back pain with numbness in the groin or buttocks, or new bladder or bowel problems, needs urgent medical care. A back support isn't a substitute for assessment.

  1. The effect of lace-up ankle braces on injury rates in high school basketball players

    1,460 high-school basketball players from 46 schools were randomized to lace-up ankle braces or no braces for a season. Braces cut the rate of acute ankle injuries by about two-thirds (hazard ratio 0.32), with or without a previous sprain, but didn't reduce the severity of injuries that happened.

  2. Treatment and prevention of acute and recurrent ankle sprain: an overview of systematic reviews with meta-analysis

    Across 46 systematic reviews, there was strong evidence that bracing, and moderate evidence that neuromuscular (balance) training, prevents repeat ankle sprains.

  3. Lumbar supports for prevention and treatment of low back pain

    Across 15 randomized trials, there was moderate evidence that lumbar supports are no more effective than no intervention or training for preventing low back pain. It was unclear whether they help treat it, and study quality was generally low.

  4. Compression Garments and Recovery from Exercise: A Meta-Analysis

    Across 23 studies, compression garments gave small overall benefits for recovering performance after exercise, and larger ones for strength recovery after resistance exercise. Whether pressure level or training status mattered was unclear.
