Serum: does it work?

A light, concentrated delivery vehicle for an active. Worth buying only for an active with evidence, at a disclosed strength.

What it is

A serum is a thin water- or oil-based product meant to carry a higher concentration of an active ingredient than a cream. The format itself does nothing special. What matters is which active is in it, at what strength, and whether it stays stable in the bottle.

The actives with controlled human trials behind them are few: retinoids (retinol, retinal, and prescription tretinoin), vitamin C as L-ascorbic acid, niacinamide, azelaic acid, and exfoliating acids. Hydrating serums built on hyaluronic acid or glycerin are simply light moisturizers. Most serums in the aisle are built around extracts, peptides or ferments with little independent evidence. How strong is the evidence?

Moderate evidence

Evidence depends entirely on the active. Tretinoin has vehicle-controlled trials for sun-damaged skin going back to 1988, and retinol reduced fine wrinkles versus vehicle in a small 24-week trial. Niacinamide trials show modest gains in blotchiness and fine lines over 12 weeks, mostly manufacturer-funded. Vitamin C only reaches skin from acidic formulas, and many derivatives did not raise skin vitamin C in lab tests. For most botanical, peptide and ferment serums, there are no good trials.

Retinoid serums reduce wrinkles

Holds up

Tretinoin has the best evidence; retinol works more slowly and gently, with smaller trials. Expect 3 to 6 months.

Vitamin C serum brightens and protects

Mixed

Modest effects in small trials, and only for stable, acidic L-ascorbic acid formulas. It never replaces sunscreen.

Niacinamide evens tone and redness

Holds up

Trials at about 5% show modest improvements over 8 to 12 weeks; most were funded by manufacturers. Higher percentages mainly add irritation.

Hyaluronic acid adds lasting plumpness

Unproven

It hydrates the surface temporarily, like any humectant; it does not replace lost volume.

Peptide and growth-factor serums rebuild collagen

Unproven

Mostly lab and small company-run studies with self-rated outcomes.

Visible results in days

False

Any change from a real active takes weeks to months; instant effects are hydration or light-reflecting particles.

What to look for

  • One evidence-backed active, named with its percentage. A concentration on the label tells you more than a long list of extracts.
  • Retinol roughly 0.1 to 1%, starting low; retinal or prescription tretinoin if you want stronger effects.
  • Vitamin C as L-ascorbic acid at about 10 to 20% in an acidic formula, in opaque or airless packaging. Discard it once it turns dark orange-brown.
  • Niacinamide at 2 to 5%; higher strengths add irritation risk without clear extra benefit.
  • Fragrance-free, with a short ingredient list, so you can tell what caused a reaction.
  • Opaque, airless or pump packaging for retinoids and vitamin C, which degrade with light and air.

What to skip

  • Stacking several actives at once. It multiplies irritation and makes reactions impossible to trace.
  • Serums sold on 'stem cells', exotic ferments or trademarked complexes without independent trials.
  • Very high niacinamide (10% and up) or very high acid strengths as a 'stronger is better' upgrade.
  • Paying for 'clinically proven' when the proof is a small company panel rating how their skin 'felt'.

How to use it

  • Add one new active at a time and wait 2 to 4 weeks before adding another.
  • Retinoids at night, starting 2 to 3 nights a week with a pea-sized amount for the whole face.
  • Vitamin C in the morning under sunscreen.
  • Apply thinnest to thickest: serum first, then moisturizer, then sunscreen in the morning.

Who should be careful

  • Avoid retinoids (retinol, retinal, adapalene, tretinoin) if pregnant, trying to conceive or breastfeeding, unless a clinician approves.
  • Retinoids and acids make skin more sun-sensitive; wear sunscreen daily while using them.
  • Expect dryness and peeling for the first 2 to 4 weeks of a retinoid; cut frequency rather than stopping. Stop and see a clinician for swelling, blistering or a spreading rash.
  • Patch test new serums on the inner arm for 7 to 10 days if you react easily.

  1. Topical tretinoin improves photoaged skin. A double-blind vehicle-controlled study

    In a 16-week double-blind trial of 30 adults, tretinoin-treated forearms improved while vehicle-treated forearms did not, and 14 of 15 faces treated with tretinoin improved versus none on vehicle. Side effects were limited to irritation.

  2. Improvement of naturally aged skin with vitamin A (retinol)

    In 36 elderly adults (mean age 87), 0.4% retinol applied to one arm up to 3 times a week for 24 weeks reduced fine wrinkling versus vehicle, with increased collagen markers in biopsies.

  3. Niacinamide: A B vitamin that improves aging facial skin appearance

    50 women applied 5% niacinamide to half the face and vehicle to the other for 12 weeks; the niacinamide side showed less fine lines, hyperpigmented spots, red blotchiness and sallowness. The study was run by a manufacturer.

  4. Topical L-ascorbic acid: percutaneous absorption studies

    L-ascorbic acid entered skin only when formulated below pH 3.5, with absorption peaking at 20%. Magnesium ascorbyl phosphate, ascorbyl palmitate and dehydroascorbic acid did not raise skin vitamin C levels.
