The fixed essential-oil blend in classic antiseptic mouthwash. Well-tested as an add-on to brushing for plaque and gingivitis; poisonous if a child drinks it.
What it is
These three plant-derived compounds (thymol from thyme, eucalyptol from eucalyptus, methyl salicylate, the main part of wintergreen oil) are combined with menthol at fixed low levels (about 0.04-0.09% each) in the classic antiseptic mouthwash. Together they disrupt the bacterial film on teeth and gums.
The evidence is for the combination as a rinse, not for any one of these compounds alone. Classic versions contain roughly 20-27% alcohol; alcohol-free versions exist.
How strong is the evidence?
Strong evidenceA meta-analysis of 29 six-month placebo-controlled trials found the rinse, added to brushing and flossing, reduced gingivitis by about 16% and plaque by about 28%; all trials were industry-sponsored. Independent systematic reviews agree: in a network meta-analysis it ranked highest among rinses for gingivitis, and it matched chlorhexidine for gum inflammation over the long term, though chlorhexidine controls plaque better.

Who should be careful
- Keep out of reach of children: swallowing a large amount is dangerous because of the alcohol and the salicylate (related to aspirin). Pure wintergreen oil is highly toxic even in small amounts.
- Follow the age limit on the label; young children cannot reliably spit.
- People avoiding alcohol (recovery, certain medications, religious reasons) should choose an alcohol-free version.
- The sting and a temporary taste change are common; stop if the mouth becomes sore or ulcerated.

29 six-month, placebo-controlled trials: adding an essential-oil rinse to brushing and flossing reduced whole-mouth gingivitis by about 16% and plaque by about 28%; 45% of rinse users versus 14% of controls had at least half their gum sites healthy.

53 six-month trials. Among rinses, essential oils, chlorhexidine (0.10% or more) and cetylpyridinium chloride (above 0.05%) had the largest effects on gingivitis, with essential oils ranking highest; active toothpastes differed little from each other, with baking soda among the weakest.

19 studies: over 4 weeks or more, chlorhexidine controlled plaque better, but the essential-oil rinse matched it for reducing gum inflammation, making it a reasonable long-term alternative.

Six-month trials showed a clinically meaningful anti-gingivitis effect for stannous fluoride pastes, the largest body of support (21 studies) for essential-oil rinses, and variable results for cetylpyridinium chloride rinses depending on the formula. Unpublished manufacturer studies were included.
