Deodorant & antiperspirant: does it work?
Deodorant masks odor; antiperspirant reduces sweat. The cancer and Alzheimer's scares haven't held up; irritation and fragrance allergy are the real issues.
What it is
Fresh sweat is nearly odorless. Underarm smell comes from skin bacteria breaking down compounds in sweat from apocrine glands. A deodorant is a cosmetic that curbs those bacteria or masks the smell, using alcohol, antimicrobials, fragrance, acids, magnesium or baking soda. An antiperspirant is an over-the-counter drug in the US: aluminum salts form temporary plugs in sweat ducts and must reduce sweat by at least 20% in standardized tests (30% for 'extra effective'). Many products are both.
Genes matter too: a common variant of the ABCC11 gene, frequent in East Asian populations, almost eliminates the typical underarm odor.
How strong is the evidence?
Moderate evidenceAntiperspirants reduce sweat by a tested, regulated amount. A population case-control study of 813 women with breast cancer and 793 controls found no link with antiperspirant or deodorant use, and the US National Cancer Institute reports no established link. One smaller case-control study reported higher risk with very frequent use from a young age, based on recalled use. Evidence that 'natural' deodorants perform as well, or that switching requires a 'detox', is lacking.

What to look for
- Antiperspirant (aluminum chlorohydrate or similar, listed under Drug Facts in the US) if wetness is your concern; deodorant alone if only odor bothers you.
- Fragrance-free if you have sensitive skin or recurring underarm rashes.
- Baking-soda-free formulas if you've had redness or darkening with 'natural' deodorants.
- Clinical-strength antiperspirants for heavier sweating before moving to prescription options.
What to skip
- 'Detox' underarm masks and cleanses.
- Paying extra for 'aluminum-free' as a safety upgrade; it's a preference, not a proven safety gain.
- Crystal or 'mineral salt' deodorants sold as aluminum-free; alum is itself an aluminum salt.
How to use it
- Apply antiperspirant to clean, completely dry skin at bedtime; reapply deodorant in the morning if you like.
- Give a new antiperspirant a few nights to reach full effect.
Who should be careful
- People with kidney disease should ask a doctor before using antiperspirants, as US labels say, because weak kidneys clear aluminum poorly.
- Don't apply to broken or freshly shaved, irritated skin; wait or switch to a gentler product.
- A rash that keeps coming back where you apply a product may be fragrance or preservative allergy; a dermatologist's patch test can pinpoint it.
- See a clinician if sweating soaks through clothes or disrupts daily life (it's treatable), or for new night sweats, especially with fever or weight loss.

Underarm odor forms when bacteria act on precursors secreted by apocrine glands. A common ABCC11 variant, frequent in East Asian populations, nearly abolishes secretion of the main odor precursors. The work was done by a personal-care company.
